This is a cute little slip to send home with a child who has lost a tooth at school. Losing a tooth is an exciting time for a young child and having this little memento makes it even more memorable.

There is a space to write the childs name and a space for the date in the bottom right corner.

1. Give it as a slip printed onto cardstock.
2. Glue it to the front of a small envelope so the tooth can be sealed inside.
