About the resource
This reusable resource helps students practise retrieval skills in order to complete a name and address form independently. The activity consolidates the use of capital letters for names and place names in an engaging way. As the resource is laminated, it allows students to edit mistakes easily. The activity is suitable for KS3 intervention groups and EAL students who are working
towards Entry Level objectives.

• This resource is perfect for:

How to use
• The Form Filler resource needs to be printed out and the individual character forms need to be cut out and laminated.
• Students will select a laminated sheet, read the information in the character’s speech bubble and extract the correct information to fill in the form.
They will enjoy choosing a name for the character!
• Extension Ideas:
✓ Students could create their own character speech bubbles containing all the relevant information, which another student can use to fill in a form.
✓ Students could fill in a form with information and another student could use
the form to create a speech bubble.
✓ EAL students could use a completed form as a conversation starter.
